We've had this happen a couple times now where our users setup an out of office rule (in their Out of Office Assistant) to reply to all email (internal and external) and the user is also in one of our approval processes.
We have the email response functionality turned on. When they receive an email from salesforce, their out of office replies to the sender. Salesforce can't make sense of the reply (APPROVED, DENIED, YES, NO, etc.) so another email is sent to the user, then their out of office replies again....on and on...
We had one user receive 500,000 emails in the span of a couple days. Just now we had our Europe Exchange server grind to a halt because of the same occurance.
Need to put in a feature into the system to detect out of office replies, and sever this loop.
