Have you explored the Google Talk integration? It is incredibly easy to setup and may be a good way to begin here.

I've heard great things about google talk, unfortunately our security won't allow it, and our IT department refuse to negotiate on that.
Whilst not ideal, Salesforce 'Ideas' or a similar custom object can be set up like a forum to accommodate questions, discussion and answers.

You could put a Chat box on the left-hand side, under Recent Items for most users, and then accessed by a Tab by the Salesforce Admin. If a user has to click on a Tab to initiate chat, they will lose their place (as far as the record in question), so it would be better if they can chat and see the problem at the same time.
